Medication Administration

 

Medication Administration at Celina ISD

  • Celina ISD does not provide any medication to students. All medication given at school must be parent or guardian provided and accompanied by a Medication Request Form. This includes prescription and over the counter medication.
  • A separate form is required for each medication.
  • A new form is required at the beginning of the school year and every time medication dosages are changed.
  • All forms related to medication and/or specific health conditions can be found on this website under the forms tab.
  • The school will not administer expired medications or medication at a dosage that exceeds the recommended maximum dosage.
  • Metered dose inhalers (MDI) are preferred over nebulized treatments in the school setting. If a nebulizer is required, we will ask for a physician's order before administering at school.

Transporting Medication to and from School

  • Parents/guardians must transport all medication. Students should not have any medication in their backpacks or lunches.
  • Controlled medication will be counted upon arrival to the clinic and documented by clinic staff and the parent.
  • Any unused, discontinued or expired medication will be disposed of at the end of the school year or 2 weeks after the last prescribed dose if not picked up from the clinic by the parent.
  • The school will not accept more than a 30 day supply of prescription medication at one time.

Parent Provided Medication

  • All medication brought to the school, including over the counter medication, is required to be in the original, properly labeled container.
  • For prescription medications, if there is a change in the prescribed dosage, a new pharmacy label must be obtained for the medication bottle.
  • The pharmacy label must correspond with the information completed by the parent on the Medication request form.
  • All medications are kept in a locked cabinet in the school clinic. The school nurse, clinic aide and CISD designated staff trained to administer medication will distribute medications.
  • A student may self-carry and self-administer an inhaler or epinephrine only after a Parent/Physician Request for Self Administration form has been completed by the parents AND the physician. This form must be renewed each school year. Please visit the forms tab to find the appropriate form.
